How do I make a referral?

Early Learning and Child Care programs as well as other agencies can make a referral by:

1. Having the parent complete the SNAP Consent to
    Release Information form (see below).

2. Calling the SNAP Coordinator at: 519-741-0076.

 The SNAP Coordinator will collect the necessary information and forward the referral to the appropriate agency/agencies.
